Parents who enroll students in the Queen of Heaven Academy Program under the age of thirteen (13) do so voluntarily. By signing this privacy policy, parents agree that Queen of Heaven Academy may collect the following personal student information to manage educational services.

3. Address
4. Phone number
5. Date of birth
Queen of Heaven Academy does not knowingly collect personal identifiable information from children under the age of thirteen (13) without verifiable parental consent. If it is determined that such information has been inadvertently collected on anyone under the age of thirteen (13), we shall immediately take the necessary steps to notify the parent and ensure that such information is deleted from our system's database. Anyone under the age of thirteen (13) must obtain parent or guardian permission to use our websites through the use of this signed Terms of Service Agreement.

Academic Records and Release of Student Information
QHA keeps all academic records confidential and will release transcripts when requested by the parent in writing via email or signed release form. Standard academic procedure is to send official transcripts from institution to institution. A student’s personal information is never sent to another student. Parents may request the parent email address be sent to another parent so their students can communicate outside of QHA as supervised and
determined by all parents. Students do not need an email address to participate in the academy since all communications is maintained internally within our course management system.

Unsubscribe or Opt-Out

All users and/or visitors to our website have the option to discontinue receiving communication from us and/or reserve the right to discontinue receiving communications by way of email or newsletters. To discontinue or unsubscribe to our websites please unsubscribe using the electronic unsubscribe tool or send an email that you wish to unsubscribe to [email protected]. If you wish to unsubscribe or opt-out from any third-party website, you must go to that specific website to
unsubscribe and/or opt-out.

Queen of Heaven Academy shall endeavor and shall take every precaution to maintain adequate physical, procedural and technical security with respect to our offices and information storage facilities so as to prevent any loss, misuse, unauthorized access, disclosure or modification of the user's personal information under our control.
